Complete skill system redesign: Add all upgrade trees, special effects, and comprehensive tests

- Added upgrade trees for ALL skills with max > 1
- Added 40+ new special effects for upgrades
- Created 38 new comprehensive tests for skill system
- Updated docs/skills.md with full documentation
- All new tests pass, lint clean
